CFIA Form 5272, Request for Release Approval

 

Please note that we have made some minor changes to the CFIA form 5272.

1- You now have to choose the Import Service Centre and the information transfer mode from a selection list.

2- The title was changed from "Request for release approval" to "Request for documentation review".  The information requested is the same. 

3- There is an additional sentence at the bottom of the form mentioning: "This review does not void any other inspection requirements."
