Update to Form 3369 – Application for Export Inspection and Phytosanitary Certification / Mise à jour du formulaire 3369 – Demande d'inspection des exportations et de certification phytosanitaire

Please be advised that the Canadian Food Inspection Agency (CFIA) has updated form CFIA/ACIA 3369 – Application for Export Inspection and Phytosanitary Certification. Exporters should be using the most current version of the form when submitting their request.

  • Digital signature capability has been added to facilitate signing and submitting this form by email to a CFIA Plant Health office.
  • The signature box has been moved to Section 8: Attestation, and must be signed by the exporter or applicant before submission to a CFIA Plant Health office.
  • The requirement for signature is stated in Directive 99-06: Policy on the Issuance of Phytosanitary Certificates for Export and for Re-export and a reference link to this policy has been added to the form instructions.
  • Applications with a signature in ink or with digital signature will be accepted. Applications will be considered invalid if submitted to the CFIA without a valid signature.

Veuillez prendre note que l'Agence canadienne d'inspection des aliments (ACIA) a mis à jour le formulaire ACIA / ACIA 3369 – Demande d'inspection des exportations et de certification phytosanitaire. Les exportateurs devraient être d'utiliser la version la plus récente du formulaire au moment de présenter leur demande.

  • Les demandes avec une signature à l'encre ou avec la signature numérique seront acceptées. Les demandes seront considérées comme invalide si présentées à l'ACIA sans signature.
